Ambulatory rehabilitation programs are designed to meet the specific needs of patients suffering from loss of autonomy, while enabling them to remain in their usual living environment. They are based on four fundamental objectives:

Coordinating care around the patient
Thanks to an interdisciplinary approach, each healthcare professional works in a complementary way to ensure seamless, coherent care focused on the patient's priorities. This coordination optimizes the efficiency of the care pathway.

Improving independence and quality of life
Our interventions help patients to regain their functional abilities, boost their confidence in everyday activities, and promote a more active, independent and serene lifestyle.

Secure return or stay at home
The program is designed to prevent avoidable rehospitalizations and facilitate a return home in the best possible conditions, with solid gains in terms of mobility, risk prevention and understanding of one's state of health.

Reducing the risk of falls and complications
By targeting the risk factors identified during the initial assessment, the care provided helps to prevent falls, limit loss of muscle mass, improve balance and support the patient in achieving lasting stabilization.

For people who have already fallen or feel unsafe walking, our programs aim to improve balance and coordination, and prevent relapses. The "Regaining confidence step-by-step" program is particularly dedicated to this problem.

Rehabilitation after hospitalization
Following surgery (fracture, severe infection, medical decompensation), a period of outpatient rehabilitation allows you to recover lost functional abilities and regain the best possible independence more quickly.

Preparation or rehabilitation for elective surgery or oncology treatment
Upstream of a scheduled operation or as part of an oncology treatment pathway, these programs help to better prepare the body for exercise, optimize recovery and limit the risk of complications.
